Dr. Max has always given a scholarship to a graduating senior at Monroe High School. This year he is opening the scholarship application process to all patients and former patients. See the application below for all of the details:
Scholarship Application
Dr. Max is offering a $500 scholarship to a graduating senior. The student must be a patient or former patient of Dr. Max and must be planning to attend a college, university, or technical school.
All applications must be received by April 19, 2012. Applicants may drop off their application at our office or mail them to:

Applications must have a cover page with the following information:
Name
High School
Anticipated major
School you will be attending
The application is a 500 word (or less) essay describing either:
What inspired you to pursue your area of study?
or
Describe a goal of yours and how do you plan to achieve that goal.
Applications will be reviewed by Dr. Max and his staff. The winning applicant will be determined by a review of their conduct as a patient and the essay. We wish all of the applicants the best of luck in their post high school endeavors.
